Complete the form. You can complete some forms on your computer and then once you have filled in all available fields, print the form and sign the form where applicable. Or you can print the form and fill everything out by hand. |

Authorization to Release Educational Records
This form can only be submitted in person at the Admissions and Records Office. Please be sure to bring a valid photo ID for both the student and the individual we are releasing information to. Copies will be made in the Admissions and Records Office. Personal copies of photo ID will not be accepted. - Request Enrollment Verification

This form can only be submitted in person at the Admissions and Records office and you must be currently enrolled in the current semester, including summer. Please be sure to bring original and appropriate photo ID/documentation. Copies will be made in the Admissions and Records Office.
